TL;DR

AZ Care Check lists every ADHS inspection of a licensed assisted living facility. In Pima County that is 1,048 inspections of active facilities, 540 with deficiencies cited. Read the latest two, check whether complaints led to citations, and ask the manager what changed.

Why does the inspection record matter so much in Arizona?

Because Arizona publishes no star rating for assisted living, the inspection record on AZ Care Check is the most detailed public view a family gets of a facility before touring. It shows what state inspectors actually found, in their own words, rather than what a brochure promises.

Our pull of AZ Care Check on October 30, 2026 covered all 310 active assisted living centers and homes in Pima County. Every one of them had at least one inspection on file, and many had five or more going back to 2023. That history is free to read and takes about ten minutes per facility once you know where to look.

What kinds of inspections show up?

ADHS labels each inspection with a type. Annual compliance inspections are the routine full reviews. Complaint inspections follow a report from a family member, resident, employee or other agency. Initial inspections happen before a new license or new owner starts operating, and modification inspections cover changes such as adding beds or a service level.

Compliance (Annual)51921341%
Compliance (Initial)6200%
Initial Monitoring39513%
Modification2000%
Change of Ownership1000%

Two patterns stand out in the Pima County numbers. Complaint inspections are common, which reflects how often families and staff use the complaint process, and a large share of annual compliance inspections cite at least one deficiency, often for paperwork or training records rather than direct care.

What does 'the following deficiencies were found' mean?

Every inspection on AZ Care Check opens with a short statement written by ADHS. 'No deficiencies were found' means the inspector found the facility in compliance with the rules reviewed. 'The following deficiencies were found' means at least one rule was not met; the citations that follow name the rule, usually a section of Arizona Administrative Code R9-10, and describe the evidence.

A deficiency is not automatically a danger. A missing signature on a tuberculosis screening form and a medication given to the wrong resident are both deficiencies. The skill is reading which rules were cited and how many residents were affected.

Which citations deserve the most attention?

Citations touching medication administration, supervision of residents who wander, falls with injury, staffing on the night shift, resident rights and abuse or neglect deserve the most attention. Paperwork citations such as an outdated policy or a missing training certificate matter less unless they repeat at every inspection.

Repetition is the real signal. A facility that is cited for the same rule at three inspections in a row has not fixed the underlying problem, even if each plan of correction was accepted.

What is a plan of correction?

After deficiencies are cited, the facility submits a plan of correction describing how it fixed each problem and how it will prevent a repeat. ADHS may accept the plan on paper or return for a follow-up review, which appears in the record as its own entry, for example an on-site review of the plan of correction where the plan was accepted for all citations.

When you tour, ask the manager to walk you through the most recent plan of correction. A good manager can explain it in two minutes and show you what changed.

How should you compare two facilities' records?

Compare like with like. A 150-bed center will have more complaints and more inspections than a ten-bed home simply because more people live and work there. Look at the share of inspections with deficiencies, how recent the last clean inspection was, and whether there is any enforcement action, which AZ Care Check lists for the last three years.

Across Pima County, 136 of the 295 facilities with a rated inspection had no deficiencies cited at their most recent one. Our directory shows that result for every facility so you can filter before you open individual records.

Frequently asked questions

Is AZ Care Check free to use?

Yes. AZ Care Check is the Arizona Department of Health Services' public portal and needs no account. Search by facility name, city or license number to see the license, service levels, inspections and recent enforcement actions for any licensed assisted living center or home.

How often does ADHS inspect an assisted living facility?

The Pima County records show annual compliance inspections for most facilities, plus complaint investigations whenever a complaint is received and initial or modification inspections when a license changes. The exact pattern for any facility is visible in its AZ Care Check inspection list.

Can I see the full statement of deficiencies?

Yes. Each inspection on AZ Care Check links to its statement of deficiencies, which lists the rules cited and the evidence. Our directory summarizes the opening line of each inspection; open the facility's record for the full text before making a decision.

Does a clean inspection mean a facility is good?

A clean inspection means no rule violations were found that day. It is a good sign, but it does not measure warmth, activities or how a particular caregiver treats your parent. Pair the record with an unannounced visit at a mealtime or in the evening.

What is a complaint investigation?

It is an inspection ADHS conducts after receiving a complaint about a facility. The record shows whether deficiencies were cited, and the complaint number is listed in the opening statement, though the complainant's identity is not.
